On the evening of 12 August 1985, Japan Airlines Flight 123, a Boeing 747 on a routine domestic flight from Tokyo to Osaka, suffered catastrophic structural failure twelve minutes after takeoff. The aircraft experienced explosive decompression that tore away much of its tail section and destroyed all hydraulic control systems. For thirty-two agonising minutes, the crew fought to control the crippled airliner using only engine thrust. The plane finally crashed into mountainous terrain near Mount Osutaka, about one hundred kilometres northwest of Tokyo.
The aircraft carried 524 people, making it one of the most densely occupied passenger flights of its time. Among them were families returning from summer holidays, business travellers, and fifteen crew members. The cause, investigators later determined, lay in a repair made seven years earlier after a minor tailstrike incident. Boeing technicians had incorrectly installed a repair plate on the rear pressure bulkhead, using a single row of rivets where two were required. Over thousands of flight cycles, metal fatigue weakened the faulty join until it finally gave way.
The impact and post-crash fires killed most passengers instantly. Rescuers did not reach the remote crash site until the following morning, despite some survivors calling for help through the night. When rescue teams arrived, they found only four survivors, all women and girls who had been seated in the rear section. In total, 520 people lost their lives, making it the deadliest single-aircraft disaster in history.
The tragedy transformed aviation safety worldwide. It led to sweeping changes in aircraft maintenance procedures, stricter oversight of repair work, and new protocols for emergency response in difficult terrain. Every year, families of the victims return to a memorial on Mount Osutaka to honour those who perished. The disaster remains a solemn reminder that in aviation, even the smallest error in maintenance can have unimaginable consequences.
